MCHA offers seven individual plans options:
$500 Deductible Plan
- Requires a $500 annual calendar year deductible ($400 medical deductible and a $100 prescription drug deductible).
- Requires a $3,000 out-of-pocket expense maximum
$1,000 Deductible Plan
- Requires a $1,000 annual calendar year deductible ($800 medical deductible and $200 prescription drug deductible).
- Requires a $3,000 out-of-pocket expense maximum.
$2,000 Deductible Plan
- Requires a $2,000 annual calendar year deductible ($1,600 medical deductible and $400 prescription drug deductible).
- Requires a $3,000 out-of-pocket expense maximum.
$5,000 Deductible Plan
- Requires a $5,000 annual calendar year deductible ($4,000 medical deductible and $1,000 prescription drug deductible).
- Requires a $5,000 out-of-pocket expense maximum.
$10,000 Deductible Plan
- Requires a $10,000 annual calendar year deductible ($8,000 medical deductible and $2,000 prescription drug deductible).
- Requires a $10,000 out-of-pocket expense maximum.
High Deductible Health Plan (HDHP)
- This is a federally qualified high deductible health plan, which allows you to open a health savings account (HSA) if certain HSA eligibility requirements are met.
- Individual and family annual deductible and out-of pocket amounts are variable, adjusted each January 1st, and are tied to the Federal annual maximum amount allowed for contribution to a Health Savings Account (HSA).
- You are responsible for the establishment of your HSA and the coordination of reimbursements.
Basic Medicare Supplement Plan
- Limited coverage for Medicare Part A and B co-payment amounts but no coverage for Part A and B deductibles, and no coverage for prescription drugs (riders can be purchased for Part A deductibles, Part B deductibles, and 80% of Usual and Customary Costs).
